• Benefit coverage is a contract between yourself, the insurance company and your employer, not the dentist.

• Ultimately, you are responsible for paying the dentist for treatment and your benefit plan will reimburse you.

• Our fees are based on clinical expertise, quality of care and materials.

• We would be pleased to complete a pre-authorization to send to your insurance company for conformation of your coverage. This will enable you to plan your finances before starting with treatment, and we would be happy to discuss fees and payment options with you.

• Often insurance companies are sending back approvals with requests for cheaper, alternative treatment plans. Our office is happy to discuss any alternative and choices with our treatment plan prior to treatment.
